New patients to Lincoln Nephrology & Hypertension will need to complete two forms ahead of your first appointment. 

The forms are PDF fillable forms but are not designed to be filled out on-line. Please download each form to your computer if you choose to fill out electronically.  The recommended app to use is Adobe PDF Reader. Important: Save your completed PDF form (use menu File > Save).
