The forms are PDF fillable forms but are not designed to be filled out on-line. Please download each form to your computer if you choose to fill out electronically. The recommended app to use is Adobe PDF Reader. Important: Save your completed PDF form (use menu File > Save).

Dialysis patients must obtain patient records from their respective dialysis center.
Payment Policy - Co-pays due at the time of appointment. If you do not have insurance coverage, payment is due at the time of appointment.
