  • What do I need to bring to my appointment?

    Please bring a list of your current medications, insurance cards and copay (if applicable).

  • What is the length of my initial consultation?

    Please arrive 15 minutes prior to your scheduled appointment time to complete any necessary paperwork. The length of your appointment may vary depending on if your doctor requests lab work. Please allow approximately one hour for your initial visit. Follow-up appointments generally last 15-30 minutes.

    Please note that your anticipated wait time may be longer than a general practitioner's office because we are a specialty clinic. We apologize in advance for any time you may have to wait to see our physicians. We will do our best to see you as soon as possible and provide you with individual attention and care.

  • How do I obtain a copy of my medical records?

  • What do I do in case of an emergency?

    Our phones are answered by our office staff from 8:00 am-4:30 pm Monday-Thursday and 8:00 am-4:00 pm Fridays. All after hour calls are transferred to our answering service. Please note that all calls after 4:00 may be answered the next business day. If a medical emergency arises after business hours please dial 911 or go to the nearest emergency room.

  • When are shots available?

    Shots are given on Fridays 8:30am-11:30am and 1:00pm-3:00pm; our office is closed 12 noon-1:00pm.

  • Do I need an appointment for lab work?

    No appointment is necessary for lab work. Lab hours are Monday-Thursday 8:00am-11:30am and 1:00pm-3:30pm; Fridays 8:00am-11:30am and 1:00pm-3:00pm.

  • Is there a copay required for lab work?

    Copays may apply. Please speak with billing concerning the requirements of your insurance.

  • Can I be billed for my copay?

    Copays are due at the time of service. If you are unable to pay, your appointment will be rescheduled to the next available time.

  • Forms of Payment

    We accept the following as forms of payment:

    Check, there will be a $10 charge on all returned checks
    Master Card

  • Insurance questions?

    If you have no insurance coverage, you may be required to pay up to $150 at the time of your visit; you will be billed for any additional charges. Payment arrangements are made in advance with our billing department.

    High deductible insurance plans will require a $100 payment at the time of service. Any balance left from your visit will be billed to you.
